APlan2Age is changing the way Americans prepare as loved ones age. As we age, we are more susceptible to serious falls and illnesses. We have choices, we can react when health events happen, or we can prepare. We teach families how to plan, through the Family Conversation followed by a Family Care Plan. The Family Care Plan can be completed online and downloaded. This Comprehensive Plan will need to be updated as circumstances change within the family. We also help families with assistance, guidance and resources when a health event has happened and a family finds themselves in crisis. We help families breathe!
